Membership Agreement

COLETTE is a private club and only members may attend functions. This agreement is a part of the club application and if you disagree with any part of these contractual provisions you will not be granted a membership or allowed to enter the club premises. Furthermore, your mere entry into the club premises constitutes your consent to these contractual provisions and you agree that you are not offended by nudity or explicit sexual behavior.

In addition to the membership fee, there will also be a user fee (door charge) for every time that a member uses the facilities. The membership fee schedule along with the user fee schedule is posted online at http://www.coletteclubs.com, and is subject to change without notice. MEMBERSHIP FEES AND USER FEES ARE NON-REFUNDABLE except as provided below.

You also understand that by signing this agreement and by paying your membership fees, you are not becoming a member until the membership committee has formally approved you. You may be allowed into COLETTE upon signing the application and paying the appropriate fees as a temporary member for that night only. During the next week the membership committee of COLETTE will discuss your becoming a permanent member. Should COLETTE decline your application for membership, then COLETTE will refund the unused portion of all membership fees paid to COLETTE if you request it in writing, and you agree that COLETTE shall retain all user fees paid in exchange for your temporary admittance and for the costs of processing your application. By providing your email during the application process, you consent to receive marketing and promotional communications from COLETTE and its affiliates. You can opt out or unsubscribe at any time by following the instructions provided in the emails. You also agree to receive SMS messages for transactional purposes, such as event reminders or membership information. Standard messaging rates may apply.

RULES AND REGULATIONS

1. "No" means "No". This is a private club for enjoyment of both men and women alike. No one has the right to disregard any other person's wishes. Therefore, should anyone decline your invitation or overtures, you shall respect that "No" and you shall not bother, annoy, harass, or persist. Enthusiastic consent is required for any form of contact. No member may touch another person or their belongings without asking first and receiving clear permission.
2. No Prostitution or Illegal Solicitation. No Illegal Drugs, No Guns or Weapons of Any Kind. If you are approached for such activity, you will notify club management immediately.
3. No Cameras, Video or Audio Equipment without written permission of the management. COLETTE is a private club and the privacy of our members is important. Talking to the press or non-members regarding this club or its members is prohibited. Doing so will be considered an invasion of the privacy of our members and our association.
4. No Inappropriate Conduct. As with "No means No" as a member of the private club COLETTE you agree to respect other members' wishes for privacy and enjoyment. Therefore, you shall not engage in any inappropriate touching or non-consensual touching, nor will you behave in a manner that is disruptive to others enjoyment. Loud, obnoxious, and inconsiderate behavior will not be tolerated. Should you act in an inappropriate manner as determined by management or should management receive complaints about your behavior that is verified by management, which management deems to be inappropriate, then you will be immediately asked to leave the premises. Should this occur more than one time then your membership will be revoked.
5. You must be over the age of 21 to become a member of COLETTE.
6. If you are asked to leave for any reason you will do so immediately. If you are asked to leave or your membership is temporarily suspended or revoked for any reason, your dues and/or fees are non-refundable.
7. Orientation. Should you have any concerns as to what might take place or what might be viewed within the private areas inside COLETTE, then before becoming a member, you should ask management any questions or concerns you might have. Should you not take the step in asking any questions you might have as to the nature of this club, then it is assumed that you know the nature of the club and approve of the same.
8. Conduct. Members will use common sense with regard to their physical safety and release, hold harmless and indemnify COLETTE, and their officers, agents, servants, employees or other members from any liability for injuries that occur as a result of the members' use of the club, its facilities, and/or equipment.
9. Membership. No one shall be allowed entry without a membership, as verified by our computer. MEMBERSHIP IS NON TRANSFERABLE
10. Relationship status. If this is a couple's membership and the status of the couple changes the membership is void. Any dues or fees are non-refundable.
11. Alcohol. COLETTE is a BYOB type of private club. No alcohol will be sold on the premises. Rather, should you desire to drink alcohol you may bring said alcohol with you to the premises, but no coolers are allowed past the door. No member shall drink alcohol to excess. Your alcohol intake is not monitored by the club and you release, hold harmless and indemnify COLETTE and their office, agents, servants or employees or other members from any damages that may occur on premise or off premise due to alcohol consumption or the alcohol consumption of other members.
12. Cell phone usage is prohibited inside the club. If you need to use your phone, please do so in the lobby
13. Sexual activity is not permitted in public areas.
14. Solicitation of phone numbers, social media accounts, or attempting to socialize with the staff of COLETTE at any time is prohibited. The staff of COLETTE is there to provide customer service and to make sure you have a clean and safe environment. Do not touch them or speak to them in an inappropriate manner
15. Offering the staff of COLETTE drugs or alcoholic beverages is prohibited. This includes on or off club premises.

The undersigned swear(s) or affirm(s) that:

1. All identifying information provided by the undersigned on the attached form is accurate.
2. The undersigned applicant(s) does/do not serve in a capacity as a law enforcement officer, investigator, inspector, agent, employee or informant of any municipal, county, state or federal governmental agency or department; In the alternative the undersigned affirm(s) that such status has been duly and fully disclosed in writing on this form.
3. The undersigned applicant(s) is/are not a reporter, employee, investigator, independent contractor or informant associated with, assisting or employed by any broadcast, print, internet or other news or entertainment media nor any organization which disseminates information in any way to the general public; in the alternative the undersigned affirm(s) that such status has been duly and fully disclosed in writing on this form.
4. The application of the undersigned for membership is made in good faith for the sole purpose of obtaining Membership at COLETTE and participating in events hosted or supported by COLETTE and not for any other purpose.

Under penalty of Perjury, I hereby state that I wish to become a member of the private club COLETTE. I further acknowledge that I have read this application form in its entirety along with the rules and regulations above. I hereby agree to abide by the terms of the application and the regulations of COLETTE and unequivocally state that no form of nudity or sexual activity offends me or will offend me and that it is my desire to become a member of COLETTE.

Shrine Agreement

If you enter and attend this function, you are responsible for maintaining current knowledge of the full list of rules located on Shrine's website https://shrineparties.com/rules-pnps/ and that you are acknowledging the rules and atmosphere of the event.

1. To enter the venue, you must be 18 or older with a current and valid State or Federal ID.
2. SHRINE REQUIRES CONSENT FOR ALL FORMS OF CONTACT. You will only touch people or other's belongings if you ask FIRST! If someone's property is inhibiting your play or posing a threat to the health and safety of others, seek a Dungeon Monitor (colored vests and/or radios). You will not engage in any non-consensual play or acts. You must receive enthusiastic consent before engaging in the play or act.
3. You acknowledge that the event safe word is RED. Saying the word RED during ANY interaction will cease the interaction IMMEDIATELY; if it doesn't, seek out a Dungeon Monitor.
4. SHRINE PROHIBITS phones, cameras, pictures, or electronic recording devices except within approved spaces (Outside, smoking area, photography room/space, and vendor booths). You will not use them in the club, including the restrooms. An Agent of Shrine will ask violators to remove the device or leave the event. Continuous violation of this rule could result in ejection from the club.
   a. If there is a suspected use of photography or video, a Shrine Agent may ask you to review the device, delete data, or both. Failure to comply will result in immediate removal and permanent banishment from all Shrine events.
5. SHRINE PROHIBITS alcohol, illegal substances (including all varieties of THC), and misuse of substances that alter your ability to control your faculties. Violating this rule could result in ejection from the club.
6. SHRINE PROHIBITS firearms, including replicas, airsoft/nerf, or fake items resembling firearms.
7. Any incidents, including but not limited to damage, injury, arguments, violations, etc., must be reported immediately to a DM, the Production Team, or both.

Under penalty of Perjury, I hereby state that I wish to become a member of the private club Colette and Shrine. I further acknowledge that I have read this application form in its entirety along with the rules and regulations above. I hereby agree to abide by the terms of the application and the regulations of Colette and Shrine and unequivocally state that no form of nudity or sexual activity offends me or will offend me and that it is my desire to become a member of Colette and Shrine.
